JUNIOR TOURNAMENT RELEASE AND WAIVER
I the GUARDIAN / PARENT/ INSTRUCTOR acting for the junior competitor named on the online entry form waive all claims against any and all persons associated with any of the participating schools and competitors. I understand the rules of the tournament and confirm that the junior competitor named on the online entry form in my care will abide by these rules. I understand that the junior competitor named on the online entry form is participating in a sport that has body contact. I assume full responsibility for all of his/her actions during and connected to the tournament.
I understand the risk of competing in this form of Martial Arts competition and on behalf of my charge, hereby release the event organizers and all of its employees and associates, tournament sponsors and the event facility, from any type of injury, loss, or death sustained while competing in this competition.
I also state that my charge is in good physical condition and know of no reason why he /
she cannot participate in this Martial Arts event. The junior competitor named on the online entry form has current NJJKC membership and/or their own valid personal insurance. I understand that divisions or weight classes are subject to change. In case of an emergency, I hereby authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary for my charge.
I also agree that the junior competitor named on the online entry form may be photographed, filmed, and/or taped and this used by the organizers, sponsors or other martial arts academies for broadcast or reproduction for any portable media. I therefore waive any compensation, thereof to the above named.
I have read the rules and the release above and agree to all of its terms and will agree to this during the online registration on behalf of the junior competitor named on the online entry form.
